Yes, if your face is sub 5 you'll never be attractive to prime women no matter how good your body is. 

BUT: going from skinnyfat/skinny/normal to in decent shape will never bring you down in points and arguably will always bring you at least some minor increase.
ALSO_1: After each major surgery you're out of training for SEVERAL WEEKS. If you go into a major surgery without having a body that will still be in that brad-pitt-in-fightclub shape after a month of lying in bed you're coping.
ALSO_2: Eyearea-losses due to lower bodyfat are COPE. You're not losing bone, the structure to hold the soft-tissue is obviously there for you. If some volume in softtissue is lost you can always replace it with fatgrafts or dermal fillers, that's what they're fore in the first place.
ALSO_3: The costs for gear is MINOR compared to ANY major surgery. Even if you live in cuckoldmany and have to get EVERYTHING (gear, fin, AI, PCT) though illicit channels a beginner cycle will cost you 200 AT MOST. You could run just the gear and skip the AI, DHT-blocker and PCT if you're running a low dose (which puts your T higher than a 95%percentile slayer's natural T) and get a cycle for 50 bucks. Even cope procedures like chin implants cost 4-digits. Every worthwhile osteotomy costs SEVERAL THOUSANDS with the top tier stuff like infraorbital+BOB, bimax, genio+rhino, wraparound jaw implants etc typically costing 5 digits.
